An endocrine surgeon assesses hormone-producing glands that may need an operation: the thyroid, the parathyroid glands and the adrenal glands. In Bahrain, most referrals are for thyroid nodules, goitre, an overactive parathyroid gland with high calcium, or an adrenal mass found on a scan. The consultation reviews your ultrasound, blood tests and any biopsy result, then explains whether monitoring, medical treatment or surgery fits your case.

What is the difference between an endocrinologist and an endocrine surgeon?

An endocrinologist is a physician who diagnoses and medically manages hormone disorders — for example an underactive or overactive thyroid, diabetes or osteoporosis — using medication and long-term follow-up. An endocrine surgeon is a surgeon subspecialised in operating on the thyroid, parathyroid and adrenal glands.

The two roles overlap and often work together. You are usually referred to an endocrine surgeon when a nodule looks suspicious on ultrasound or biopsy, when a gland is large enough to cause pressure symptoms, when an overactive gland does not respond to medical treatment, or when an adrenal mass is hormonally active or growing.

Thyroid conditions

  • Single or multiple thyroid nodules found on examination or ultrasound.
  • Goitre and multinodular goitre causing pressure, discomfort or a visible swelling.
  • Nodules with indeterminate or suspicious biopsy results.
  • Thyroid cancer requiring lobectomy or total thyroidectomy.
  • Overactive thyroid that has not been controlled with medication or radioiodine.

Parathyroid and adrenal conditions

Primary hyperparathyroidism is usually discovered through a raised calcium level on a routine blood test, sometimes with fatigue, kidney stones or bone pain. Localisation studies help identify the overactive gland before surgery.

Adrenal masses are often found incidentally on a CT scan. Assessment focuses on whether the mass produces excess hormones and whether its size or imaging features make surgery advisable.

How the assessment works

  • History and neck examination.
  • Review of thyroid function tests, calcium and other relevant hormone results.
  • Neck ultrasound review, with fine-needle aspiration when a nodule meets biopsy criteria.
  • Cross-sectional imaging where indicated for large goitres or adrenal masses.
  • A written plan setting out follow-up intervals or the surgical option and its risks.

Frequently Asked Questions

What is the difference between an endocrinologist and an endocrine surgeon?

An endocrinologist manages hormone disorders with medication and follow-up. An endocrine surgeon operates on the thyroid, parathyroid and adrenal glands when surgery is indicated. Many patients see both.

Which gland problems usually need surgery?

Suspicious or cancerous thyroid nodules, large goitres causing pressure symptoms, primary hyperparathyroidism, and adrenal tumours that are hormonally active, large or growing.

Does a thyroid nodule mean I need an operation?

Most nodules are benign and are monitored with ultrasound. Surgery is considered when the biopsy is suspicious, the nodule grows significantly, or it causes pressure symptoms.

What should I bring to an endocrine surgery consultation?

Your thyroid function tests, calcium and vitamin D results, neck ultrasound report and images, any biopsy result, and a list of current medications.

Will I need hormone tablets after gland surgery?

It depends on the operation. Removing the whole thyroid means lifelong thyroid hormone replacement; after a lobectomy many patients need none. This is discussed before surgery.
