Hemorrhoids Assessment and Treatment in Bahrain

Dr. Haneen AlBoosta — Consultant General, Endocrine & Laparoscopic Surgeon in Bahrain

Hemorrhoids are enlarged vascular cushions in the anal canal that commonly cause painless bright red bleeding, itching, discomfort or a lump. Assessment involves a discreet examination, grading of internal hemorrhoids from I to IV, and identifying contributing factors such as constipation, straining or pregnancy. Rectal bleeding is never assumed to be hemorrhoids without an examination, since other conditions can produce identical symptoms.

Internal and external hemorrhoids

  • Internal hemorrhoids arise above the dentate line and are usually painless, presenting with bleeding or prolapse.
  • External hemorrhoids arise below it and can be painful, particularly when a clot forms (thrombosis).
  • Both types can be present at the same time.

Grading of internal hemorrhoids

  • Grade I: bleeding without prolapse outside the anal canal.
  • Grade II: prolapse during a bowel movement that reduces spontaneously.
  • Grade III: prolapse that requires manual reduction.
  • Grade IV: permanently prolapsed and irreducible.
  • Grading guides treatment: lower grades usually respond to conservative or office-based measures.

Common symptoms and contributing factors

  • Bright red blood on the paper, in the toilet bowl or coating the stool.
  • Itching, irritation or a moist sensation around the anus.
  • A lump or swelling, sometimes tender.
  • Chronic constipation, straining and prolonged sitting on the toilet.
  • Pregnancy and the postpartum period, from pressure and hormonal change.
  • A low-fibre diet, inadequate fluid intake and limited physical activity.

Hemorrhoids in pregnancy

Hemorrhoids are common in pregnancy and after delivery. Treatment focuses on fibre, fluids, safe stool softeners and topical measures approved in pregnancy. Many cases improve significantly in the months after delivery, and procedures are usually deferred unless symptoms are severe.

Rectal bleeding must always be assessed

Anal fissures, inflammatory bowel disease, polyps and colorectal cancer can all cause bleeding that resembles hemorrhoids. Assessment is more thorough if you are over forty-five, if the bleeding is dark or mixed into the stool, if bowel habit has changed, or if there is unexplained weight loss, anaemia or a family history of colorectal cancer. Being examined is what allows the correct diagnosis to be made.

When to book an appointment

  • Bleeding that recurs or does not settle within a couple of weeks.
  • A painful anal lump or one that has appeared suddenly.
  • Prolapse during or after bowel movements.
  • Symptoms that persist despite fibre and topical treatment.

Frequently Asked Questions

Are hemorrhoids the only cause of rectal bleeding?

No. Anal fissures, polyps, inflammatory bowel disease and colorectal cancer can cause similar bleeding, which is why bleeding should be examined rather than assumed to be piles.

Is the examination uncomfortable?

It is brief and performed discreetly with local anaesthetic gel where needed. Most patients find it far less uncomfortable than expected.

Why did hemorrhoids appear during pregnancy?

Increased pelvic pressure, hormonal changes and constipation all contribute. Symptoms often improve after delivery with fibre, fluids and pregnancy-safe treatments.

Can hemorrhoids be treated during the same visit?

Some office-based treatments such as rubber band ligation can be performed in the clinic for suitable grades, after the examination and discussion.

When should I go to the emergency department?

For heavy or continuous bleeding, dizziness or fainting, or a severely painful, hard anal lump with fever.
